CONTOUR (2002)
CONTOUR (Comet Nucleus Tour) was a NASA Discovery-program spacecraft built by the Johns Hopkins University Applied Physics Laboratory to fly close by and photograph the nuclei of two, potentially three, comets. It launched successfully on July 3, 2002, and spent 43 days in a phasing orbit around Earth. On August 15, 2002, its solid rocket motor fired to push it out of Earth orbit toward comet Encke. No telemetry was designed to be collected during the burn, and no signal was received afterward. Ground telescopes soon found several objects near the spacecraft's expected position, indicating it had broken apart. The story
The ambition
CONTOUR, short for Comet Nucleus Tour, was proposed by Cornell University's Joseph Veverka as a principal-investigator-led mission for NASA's low-cost Discovery Program. Selected in August 1997 as the program's sixth mission, it was designed, built, and would be operated by the Johns Hopkins University Applied Physics Laboratory. The spacecraft carried two imagers, a mapping spectrometer, and a dust analyzer, and was built to fly within a few hundred kilometers of a comet nucleus at high resolution, close enough to image surface features and sample the surrounding coma directly. Its baseline plan called for flybys of comet Encke in November 2003 and comet Schwassmann-Wachmann 3 in June 2006, with a possible third encounter with comet d'Arrest if the spacecraft remained healthy.
The rise
CONTOUR launched on July 3, 2002, from Cape Canaveral aboard a Delta II 7425 rocket, entering a highly elliptical phasing orbit around Earth with a period of roughly 1.75 days. Over the following 43 days, the Applied Physics Laboratory operations team carried out 23 hydrazine thruster maneuvers to refine the spacecraft's orbital geometry, positioning it for the burn that would send it out of Earth orbit and on toward comet Encke. APL reported that every subsystem performed normally through this phase. The spacecraft's most consequential piece of hardware had not yet fired: a STAR 30BP solid rocket motor, built by ATK (formerly Thiokol) and embedded permanently inside the spacecraft body, positioned to push CONTOUR out of Earth orbit in a single burn.
The cracks
The mission's vulnerability was invisible before launch. CONTOUR's solid rocket motor nozzle was embedded within the spacecraft to a greater degree than was typical for this kind of motor, which meant the exhaust plume during firing would interact with the spacecraft's own structure at close range. The investigation board later found that the near-field heating effect of that plume had not been adequately analyzed in the spacecraft's design. The Applied Physics Laboratory's plume heating analysis assumed a worst-case combined radiative and convective heating environment well below what independent analyses commissioned after the loss would find, and the original design work also did not account for how plume contamination could degrade the thermal properties of the spacecraft's surface materials, which would have compounded any heating beyond what was predicted. Separately, mission planners had not built in any way to observe or receive telemetry during the burn itself, so if something went wrong during those fifty seconds, there would be no direct evidence of it.
The collapse
At 4:49 a.m. Eastern time on August 15, 2002, CONTOUR fired its STAR 30BP motor for a planned fifty-second burn meant to push it out of Earth orbit toward comet Encke. The burn's orbital geometry meant the spacecraft could not communicate with ground stations during the firing, by design, so no telemetry came back while it happened. Operators expected to reacquire the signal around 5:35 a.m. Eastern via NASA's Deep Space Network. No signal came. Active attempts to reach the spacecraft in the following days failed, and an onboard contingency sequence built to cycle through backup transmitters and antennas if contact was lost also produced nothing. From August 16 through 21, ground-based telescope images taken at the University of Arizona's Kitt Peak observatory found three separate objects on trajectories close to where CONTOUR was expected to be, and data from the Department of Defense corroborated that the spacecraft had broken apart. NASA scaled back contact attempts through the fall and, after a final round of listening in December failed, formally declared CONTOUR lost on December 20, 2002.
The aftermath
NASA convened a Type A Mishap Investigation Board on August 22, 2002, chaired by NASA Chief Engineer Theron Bradley Jr., to determine what had happened. Because no telemetry or optical observation existed from the burn itself, the board could not directly witness the failure. It instead reviewed design, manufacturing, testing, and operations records, built a fault tree of possible failure modes, and commissioned independent plume-heating, thermal, and dynamics analyses beyond what APL and ATK had originally performed. The board's May 2003 report concluded that the probable proximate cause was overheating of the spacecraft's forward end from the rocket motor's exhaust plume during the burn, likely weakening the structure enough to cause a catastrophic breakup. It could not rule out several alternate causes, including outright failure of the solid rocket motor itself, a collision with space debris, or a loss of attitude control, given the near-total absence of direct evidence. CONTOUR was the only mission in NASA's Discovery Program to be lost outright, and its loss did not lead to casualties or direct public danger; the cost fell on the roughly $150 million mission itself and the years of preparatory work behind it.
The lessons
The board's root-cause findings pointed past the plume-heating failure itself and toward how the spacecraft came to be built that way. It found that the CONTOUR project had leaned on analysis "by similarity" to previous designs without confirming that the heritage assumptions actually applied to this spacecraft's unusual motor configuration, that the Applied Physics Laboratory's systems engineering process around subcontractor requirements and internal peer review was less rigorous than comparable NASA work, and that the review process built into the Discovery Program's milestone system was too high-level to catch a design risk this specific. A single, uninspected engineering assumption, that a rocket motor's exhaust plume would behave the way previous missions' motors had, went unchallenged all the way to flight. Because the mission design also left no way to observe the one maneuver that mattered most, investigators were left reconstructing a probable cause from lab analysis rather than confirming one from data, a gap that by itself became one of the board's most pointed observations.
